Include
- Song title and date
- Each contributor’s legal name, stage name, role and contact information
- PRO affiliation and IPI number (if known)
- Publisher or administrator for each writer
- Composition share for each writer (must total 100%)
- Any agreement about the recording, if applicable
- Signatures and dates from every contributor
